The Supreme Court of India has decided to abolish the state quota for NEET PG admissions starting next year. All admissions will now fall under the All India Quota.

This decision means that state-based reservations for PG medical courses will no longer apply, with the court asserting that all candidates are considered part of India.

(Please note, this ruling will not impact the ongoing NEET PG admissions for the current year)

Legal challenges are expected, as this ruling could lead to multiple court petitions.

The given CT image shows that there are multiple heterogeneous lesions identified in both kidneys.
These are composed of macroscopic fat components along with solid enhancing areas and vessels. These findings are characteristic of angiomyolipoma.
Bilateral renal angiomyolipomas in patients with a history of seizures are suspicious of tuberous sclerosis.
Tuberous sclerosis presentes with a triad of mental retardation, seizures and adenoma sebaceum.
In this condition, there will be hamartoma in various parts of the body (in eyes, lungs, heart and kidneys).

Solutions ✅

The given sonography shows dilated renal pelvis and calyx.
The ureter is not dilated.
These findings are suggestive of obstruction at the junction of renal pelvis and ureter- Known as pelviureteric junction obstruction (PUJO).
In PUJO, the dilation of the renal pelvis will be disproportionate to ureteral dilation.
As there is obstruction, the radiotracer will get accumulated in the renal pelvis.

Solutions ✅

The given image of a micturating cystourethrogram (MCU) shows reflux in bilateral kidneys.
The bilateral ureters and renal pelvis are dilated and tortuous with associated blunting of calyx.
These findings are suggestive of grade 5 vesicoureteral reflux (VUR).
Based on MCU, the VUR has 5 grades:

Grade 1: Reflux limited to the ureter.
Grade 2: Reflux up to the renal pelvis.
Grade 3: Mild dilatation of ureter and pelvicalyceal system.
Grade 4: Tortuous ureter with moderate dilatation and blunting of fornices but papillary impressions are preserved.
Grade 5: Tortuous ureter with severe dilatation of ureter and pelvicalyceal system with loss of fornices and papillary impressions.
